Custom plans
For groups with at least five employees, you can customize the
group's plan by selecting frequency limits, types of services, deductible, calendar-year
maximums, waiting periods, orthodontia and coinsurance. The plan design
possibilities are endless!
Six plan designs
For groups of two or more, six standard plan designs
are available with calendar-year maximum benefits ranging from $250 to $1,500.
PPO and Indemnity
Flexident offers two different PPO plan arrangements and also a Traditional Indemnity option.
Employer contribution
The employer can pay a portion, all or none of the
employee premium. Participation guidelines vary based on the employer's level
of premium contribution. Self-funded plans are also available for groups with 25 or more employees.
